I want to enroll in the price cap program. I understand my price will not go above the cap and I will pay a lower amount if your daily price falls below the cap. The fee for this program is 28 cents per gallon, which will appear as a separate line item on my next statement. The full cost of the fee is due within 10 days of receipt. The cap price is currently $3.229 per gallon.

I wish to purchase gallons of fuel for the season. (You must enter a number in this box.) Refer to the letter we sent you to see how much fuel you used last year. If you require more fuel than the amount stated, you will be charged the price per gallon on the day of the delivery.

I would like to spread out my payments with EZ Pay (if you are already an EZ Pay customer your account is automatically renewed), your monthly payment program, but prefer to purchase my fuel at the daily rate. I understand my price will not be capped and will float up or down with the market. I have the option of purchasing price cap protection at a later date.
